As a carrier, the FMCSA requires that you maintain a driver qualification file on every driver, even if you drive.

I have attached a detailed checklist with some explanation on each item including a link the regulation.

Also included is a file that includes all the documents you need to complete the driver file. It is important that you take this seriously because it is going to be reviewed in your New Entrant Audit.

If you prefer to make sure that your driver file is “Audit Ready”, for $50* we will take your application over the phone, do all the required background checks, and make sure that you have all the required documents. *plus the cost of any reports that are run through our system.

Initial Driver File (brief list, review attached document):
  • Drivers Application for Employment (DOT Specific Application)
  • Driver’s Road Test Certification or Equivalent
  • Inquiry to Previous Employers
  • Safety Performance History Records
  • 3-year Driver Motor Vehicle Record (MVR)
  • Pre-Employment Drug and Alcohol Documents
